- About these terms
These Terms & Conditions apply to physiotherapy services provided by Home Health Physio, a sole trader offering home visit and care home physiotherapy services.
By booking an appointment or purchasing a package, you agree to these Terms & Conditions.
- Services provided
Home Health Physio provides physiotherapy assessment, treatment, rehabilitation, and advice in patients’ homes, care homes, and other agreed locations.
Treatment is based on clinical assessment and professional judgement.
- Appointments and home visits
- Appointments are arranged in advance.
• Home visits and care home visits will be carried out at an agreed time and location.
• It is your responsibility to ensure access to the property and a safe working environment.
- Fees and payment
- Fees will be agreed at the time of booking or prior to treatment.
• Payment is accepted by BACS transfer or cash unless otherwise agreed.
• Payment is due at the time of treatment or within agreed terms.
• This includes single appointments and pre-paid treatment packages.
- Packages and course of treatment
- Home Health Physio may offer discounted treatment packages or courses of treatment.
• All packages must be paid for in full at the time of purchase unless otherwise agreed.
• Package purchases are non-refundable once payment has been made.
• Package sessions are valid for a period of 6 months from the date of purchase unless otherwise stated.
• Unused sessions will expire after the validity period and are non-transferable and non-refundable.
• Individual sessions within a package are subject to the same cancellation policy as standard appointments.
• Home Health Physio reserves the right to discontinue a package if clinically appropriate. In such cases, any unused sessions will be reviewed on a case-by-case basis.
- Cancellations
- Appointments cancelled with less than 24 hours’ notice may be charged in full.
• Failure to attend an appointment may also be charged.
- Communication
Home Health Physio may communicate via:
• Telephone
• Email
• SMS
• WhatsApp
WhatsApp may be used for appointment coordination and general communication. It must not be used for emergencies.
- Clinical responsibility
Treatment is provided based on clinical assessment at the time of contact. Outcomes cannot be guaranteed.
Patients are responsible for following advice and exercises provided.
- Safeguarding and safety
Home Health Physio reserves the right to decline or stop treatment if a safe working environment cannot be maintained.
- Liability
Home Health Physio holds appropriate professional indemnity insurance.
Nothing in these Terms excludes liability for negligence.
- Data protection
Personal information is handled in accordance with the Home Health Physio Privacy Notice.
- Changes to terms
These Terms & Conditions may be updated from time to time. The most recent version applies to all services.
